MEDICAL PRACTICE - ENHANCED PRIMARY CARE
Enhanced
Primary Care (EPC) is when your preferred treating General
Practitioner coordinates the development of team care
arrangements, consisting of allied health practitioners,
specialist and/ or dentists, to help with successful
management and care of your health needs. In the past
Medicare has not offered a rebate on allied health services,
but with the introduction of EPC this is no longer the
case.
Your GP will work out a care plan that is best suited
to your needs in regards to managing your health needs.
Once your Enhanced Primary Care plan is in place, you
will be able to access eligible allied health and/or
dental care services identified in the plan, on referral
from your GP.
As Holdsworth House Medical Practice already has EPC
allied practitioners on site, implementing your new
care plan is as easy as asking the receptionist to book
your initial appointment for you.
In order to be able to claim your money back in a hassle
free manner from Medicare, we advise that you allow
five working days from the consult with your GP before
taking your allied health service receipts into Medicare
for processing.
For your first visit with your allied health practitioner
you will need to bring the EPC form which has been signed
and given to you by your GP and the referral from your
GP. For any consecutive visits your information will
be kept on file so you will not be required to bring
your referral each time.
